2008-09 Parking Decal Request & Payroll Deduction Form

For the 2008-09 year, the cost for an annual parking decal will be $86.00, or $43.00 per semester. The cost for an annual evening decal is $36.00, or $18.00 per semester (these decals are valid only between 3:30 p.m. and 7:00 p.m. and are not available through payroll deduction).

  1. Complete the parking decal application: 2008-09 Parking Decal Request & Payroll Deduction Form [doc]. Please include your license plate(s) AND state. Decals will not be issued without ALL VEHICLE LICENSE NUMBERS.
  2. If you are payroll deducting, complete and sign the attached payroll deduction card. Eight monthly withholdings of $10.75 each will automatically be deducted beginning in the September payroll period. The total deductions taken for the academic year will be $86.00. Payroll deduction is available only for the annual amount and only for permanent employees. THE PAYROLL DEDUCTION OPTION ENDS AUGUST 29, 2008. Note: Internal Revenue codes now exempt payroll deducted parking fees from Federal Income and Social Security taxes. Your payroll deduction will automatically take advantage of this pretax revenue code change. If you are not payroll deducting, make check payable to UW‑Green Bay.
  3. Mail these items, via campus mail, to Parking (not to the Bursar’s Office) by August 22, 2008. Your decal will be mailed to you via campus mail. You may also bring these items in person to the Bursar’s Office (SS 1300) between 8:00 a.m. and 4:30 p.m. This should be done by August 26, 2008, to avoid the lines that form during the student fee payment period.
  4. Decals must be purchased by 4:00 p.m., Friday, September 5, 2008. (Fall semester decal enforcement begins 8:00 a.m., Monday, September 8.)

Parking permits are available for purchase from the Parking Office at the rate of $5.00 per week or $1.00 per day.
