Join Healthy Sheboygan County on Thursday, October 2nd from 5pm to 6:30pm in the Phoenix Room on the UW-Green Bay Sheboygan Campus for a Community Health Improvement Plan (CHIP) update meeting to learn more about how we are making a difference.

A CHIP is developed based on the priority issues identified by community members in a Community Health Assessment (CHA). The goal of the CHIP is to improve the health of the community by working together to address the most pressing health concerns. The work that is done focuses on actions that were developed for the community, by the community. Healthy Sheboygan County’s current CHIP cycle will run from 2024 through the end of 2028.

Five priority areas were chosen for this cycle based on information gathered in the CHA:

1. Positive mental health

2. Responsible substance use

3. Activity and nutrition

4. Access to affordable housing

5. Access to affordable, high quality childcare

Each of these priority areas has a dedicated action team that meets regularly to complete the work outlined in the CHIP. The Healthy Sheboygan County Community Health Assessment and Improvement Plan can be found on our website healthysheboygancounty.org.

Healthy Sheboygan County (HSC) was formed in 1993 and is a community-based initiative seeking to make a positive impact on the health of Sheboygan County residents. Members and representation includes public health, local health care agencies, non-profit organizations, schools, businesses, and community representatives that work together to help Sheboygan County community members live better, longer lives.
